About the Role

We are seeking an experienced social worker to join our Adult Community team, dedicated to providing support and intervention for individuals aged 18 and above, living with physical disabilities, chronic conditions, autism, and sensory impairments.

Key Responsibilities
  • Collaborate with individuals, families, and caregivers to conduct Care Act 2014 assessments, evaluate needs, and manage both short-term and long-term cases.
  • Advance safeguarding inquiries, handle Court of Protection matters, and maintain close collaboration with healthcare and multidisciplinary professionals.
  • Develop and implement effective support plans, ensuring the best possible outcomes for service users.
  • Work closely with the team to ensure a high-quality service, promoting a culture of excellence and continuous improvement.
Requirements
  • Qualified Social Worker with evidence of post-qualifying learning and development.
  • Social Work England registered.
  • Proven track record of strengths-based social work with adults, including the safeguarding of adults.
  • Extensive knowledge of health and social care and other relevant legislation policies and guidance.
  • Evidenced experience of professional and personal development following achievement of professional qualification, likely to be a minimum of 2 years post-qualification.
  • Well-developed interpersonal skills and ability to effectively communicate with people in a variety of ways and levels.
  • Ability to utilise a range of Microsoft and electronic social care record software.
