About the Role

We are seeking an experienced Adult Social Worker to join our Hospital Team. As a key member of our team, you will be responsible for carrying out Care Act 2014 assessments, MCA's, and creating care packages to support patients in their transition back to the community.

Key Responsibilities:

  • Conduct thorough assessments to identify patient needs and develop personalized care plans
  • Work collaboratively with multidisciplinary teams to ensure seamless patient care
  • Develop and implement care packages to support patients in their recovery
  • Provide expert advice and guidance to patients and their families

Requirements:

  • Minimum 2 years post-ASYE experience in Adult Social Care
  • Strong knowledge of the Care Act 2014 and MCA's
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
