About the Role

We are seeking an experienced Advanced Healthcare Practitioner to join our team in Buckinghamshire. As a key member of our Adult Community Health Care Teams, you will play a vital role in delivering high-quality care to patients in their own homes.

Main Responsibilities
  • Provide skilled assessment and management of patients with complex health needs
  • Case find and intensively manage patients who are high-risk users of community and primary healthcare
  • Develop and implement systems to support intensive case management within the multi-disciplinary team
  • Work collaboratively with other agencies to prevent hospital admission and support early discharge

Person Specification
  • Educated to Degree level or equivalent in a health-related subject
  • Level 1 Registered Nurse
  • Recognised teaching or mentorship qualification equivalent to level 3
  • Recognised supervisory management qualification equivalent to level 3
Desirable Qualifications and Experience
  • District Nursing SPQ desirable
  • Independent/supplementary non-medical prescriber
