About the Role

We are seeking a highly skilled and experienced Community Case Manager to join our Southern Adult Community Healthcare Team. As a key member of our team, you will play a vital role in providing high-quality patient care and contributing to the development of our healthcare services.

Key Responsibilities
  • Contribute to the development and implementation of case management programs for patients with complex needs.
  • Assess and manage patient caseloads, providing tailored interventions to meet individual needs.
  • Collaborate with multidisciplinary teams to coordinate care and ensure seamless transitions.
  • Develop and maintain partnerships with community organizations and healthcare providers to enhance patient outcomes.
  • Provide education and support to patients, families, and caregivers to promote health and well-being.
Requirements
  • Registered Allied Healthcare Professional or Registered Level 1 Nurse.
  • Degree or Diploma in Health or Nursing Studies or related subject.
  • Evidence of recent Continuous Professional Development.
  • Relevant postgraduate clinical training in areas such as elderly care, long-term conditions, and palliative care.
