Role Context:

The Clinical Pharmacy initiative within General Practice is part of a transformative program aimed at creating a new care model that emphasizes person-centered and coordinated healthcare. This initiative aligns with national policies, including the Five Year Forward View and GP Forward View, which advocate for a more effective utilization of pharmacy roles in primary care to help patients maintain their health and avoid hospital admissions while alleviating pressures on general practice.

Key Responsibilities:

The Pharmacy Technician will support the Practice-Based Clinical Pharmacist in delivering safe, effective, and efficient systems for repeat prescriptions, optimizing medication use, minimizing waste, and enhancing patient outcomes.

  • Conduct medication reconciliation in line with established practice protocols.
  • Assist in achieving Quality and Outcomes Framework (QoF) targets.
  • Collaborate with relevant personnel within the GP practice to review and enhance repeat prescribing protocols and Standard Operating Procedures (SOPs).
  • Provide guidance to GPs, staff, and patients regarding prescribing changes to improve safety, quality, and cost-effectiveness.
  • Engage in medication audits.
  • Support the monitoring and management of prescribing budgets, liaising with the medicines management team.
  • Develop evidence-based resources to facilitate rational and cost-effective prescribing.
  • Conduct medication reviews and community visits as necessary.
  • Proactively manage the safe handling of prescriptions, assessing risks and recommending improvements.
  • Address medication inquiries from patients and staff, referring to the appropriate GP as needed.
  • Advise on the sourcing and management of medications.
  • Assist in submitting claims for personally administered drugs.

Qualifications:

Essential:

  • NVQ Level 3 in Pharmacy Services (or equivalent).
  • Registered Pharmacy Technician with the General Pharmaceutical Council.
  • Evidence of Continuing Professional Development (CPD).
  • Understanding of prescribing and medicines management.
  • Experience working effectively within multidisciplinary teams.
  • Ability to engage with the public and patients.

Desirable:

  • Post-qualification experience.
  • Experience in NHS Primary Care.
  • Familiarity with EMIS clinical systems.
  • Proficiency in Microsoft Office applications.
