Job Summary

Apollo Home Healthcare is seeking an experienced Care Coordinator to join our dedicated team in the West Midlands region. As a skilled Care Coordinator, you will play a key role in the continuous delivery of care to service users, managing a caseload of care packages and understanding the varied nature and fast-pace of the role.

Key Responsibilities

  • Manage rotas for care teams, respond efficiently to day-to-day changes, and act as the first point of contact for service users, their families, and other healthcare professionals.
  • Apply communication and relationship-building skills to effectively manage care delivery, ensuring seamless coordination and high-quality patient care.
  • Work collaboratively with healthcare professionals to develop and implement personalized care plans, ensuring the best possible outcomes for service users.
  • Provide exceptional customer service, responding to queries and concerns in a timely and professional manner.
  • Contribute to the development and implementation of care policies and procedures, ensuring compliance with regulatory requirements.

Requirements

  • Proven experience as a Care Coordinator or in a similar role, with a strong understanding of care delivery and patient needs.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and healthcare professionals.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Strong problem-solving skills, with the ability to think critically and make sound judgments.
  • Valid driving license and access to a vehicle, with the ability to travel across the West Midlands region.

  • Competitive salary between £29,700 - £34,700 per annum, including payment for on-call duties.
