Diabetes Specialist Nurse

We are seeking a highly motivated and adaptable Band 7 Diabetes Specialist Nurse to join our established Diabetes team within the acute trust tier of our integrated Diabetes service.
The successful candidate will cover the weekend service on a Saturday and Sunday between the hours of 08:00-16:00, and one weekday from 08:00-16:00, which can be discussed and agreed upon with the diabetes nursing clinical lead.
As a key member of our diabetes team, you will provide specialist clinical nursing support, advice, and education to patients and their families. You will work effectively with the multidisciplinary teams both in the hospital and wider community.
We are looking for a candidate with evidence of recent diabetes clinical practice and experience. The successful candidate will be adaptable and flexible to meet the challenges of this role.
The main role of the post holder will be to provide specialist support to inpatients with diabetes, including triage referrals, education, review, titration, and initiation of medication. You will also participate in nurse-led telephone and email clinics, and provide skilled assessment, planning, delivery, and evaluation of care within defined guidance and protocols.
As a recognised expert in diabetes, you will be responsible for assessing, developing, and implementing specialist nursing care programmes for patients with diabetes. You will also supervise nursing staff and provide multi-professional education in diabetes care.
The post holder will be involved in the development and implementation of clinical protocols and guidelines for inpatients and outpatients with diabetes. They will work towards developing a structured approach in diabetes care to ensure a comprehensive high-quality diabetes service with the Trust and community tier of the diabetes services.
