Advanced Clinical Practitioner Opportunity

We are seeking an experienced Advanced Clinical Practitioner to join our team at Primary Care 24, a not-for-profit social enterprise delivering NHS contracts exclusively.

About the Role

As an Advanced Clinical Practitioner, you will be responsible for assessing, diagnosing, and providing advice, care, and management to patients presenting with undiagnosed or undifferentiated illness via telephone consultations or face-to-face assessments for patients requiring home visits.

Key Responsibilities
  • Undertake face-to-face and telephone triage consultations
  • Interpret pathology/radiology results/reports
  • Prescribe and review medication
  • Plan, implement, and evaluate an effective management plan for the patient
Requirements

You will be an experienced Advanced Clinical Practitioner who is able to deliver quality individualized care to our patients with a high level of autonomy underpinned by the four pillars of Clinical practice, leadership and management, education, and research.
