Job Summary

We are seeking a highly skilled and dedicated Recovery Navigator to join our team at Humankind. As a key member of our Calderdale Recovery Steps Team, you will play a vital role in supporting individuals with substance misuse issues and promoting positive community reintegration.

Key Responsibilities
  • Prison In-Reach and Community Support: Provide support to individuals in prison to facilitate positive community reintegration, including linking with community support organizations.
  • Collaboration with Probation and Integrated Offender Managers: Work closely with Probation and Integrated Offender Managers to ensure continuity of care and support.
  • Development and Delivery of Interventions: Lead on the development and delivery of bespoke interventions to support offenders in their recovery journey.
  • Caseload Management: Manage a caseload by creating, implementing, and reviewing ongoing recovery and treatment plans.
  • Facilitating Progression: Facilitate and promote the progression of individuals from initial engagement into structured treatment.
  • Reducing Substance-Related Harm: Contribute to reducing substance-related harm to individuals and the wider community.
  • Collaboration with Key Partners: Collaborate closely with key partners and colleagues to achieve the best health and wellbeing outcomes for individuals.
  • Co-Production Principles: Apply co-production principles to any stage of treatment and recovery, ensuring excellent boundaries and a person-centered approach.
  • Creative Treatment Options: Think creatively about treatment options, changes, and transitions in the context of a multi-disciplinary and multi-agency approach.
  • Service User Engagement: Encourage service users to become involved in the service and in service user forums in Calderdale, enhancing their experience and increasing a sense of belonging.
Requirements
  • Substance Misuse-Specific Knowledge: Provide substance misuse-specific knowledge and interventions, offering the right treatment to people, where they are at.
  • Treatment Intensity and Pace: Ensure that ample treatment intensity is provided, at the appropriate pace, and think ahead to make sure that doors are opened for enhanced recovery and mutually agreed successful outcomes are achieved.
  • Strength-Based Approach: Work alongside other frontline practitioners using a Strength-Based Approach, focusing on individuals and community assets.
  • Resourcefulness and Communication: Be resourceful, able to articulate yourself appropriately on behalf of your service users, and resilient to change and challenges.
