Community Health Nurse

We are seeking a highly skilled and dedicated Community Staff Nurse to join our team in the Bromley area. As a Community Staff Nurse, you will play a vital role in providing out-of-hospital services to patients, working closely with our team to deliver high-quality care.
Key Responsibilities:- Contribute to the holistic assessment, planning, delivery, and evaluation of patient care
- Implement, monitor, and maintain high standards of care
- Re-assess and re-evaluate programmes of care when needed
- Be an active member of the team, sharing best practice and contributing to clinical delivery
- Assist with daily coordination of the team, including daily handover, allocation of work, and managing referrals and telephone messages
- Qualified as a Registered Nurse with a current active NMC Pin
- Experience of writing accurate records
- Evidence of provision of high standards of clinical care
- Experience in administration of drugs or willingness to attend study days
- Knowledge of standard setting/clinical audit
- Community experience/community placement
- Excellent salary of £38,768 - £46,124 per annum, Band 5 and Band 6 available inclusive HCAS
- 37.5 hours per week, permanent full-time role
