Community Health Nurse

Job Summary:
Jupiter Recruitment is seeking a highly skilled and dedicated Community Staff Nurse to join our team. As a Community Staff Nurse, you will be responsible for providing high-quality patient care in an out-of-hospital setting.
Key Responsibilities:
- Contribute to the holistic assessment, planning, delivery, and evaluation of patient care.
- Implement, monitor, and maintain high standards of care.
- Re-assess and re-evaluate programs of care when needed.
- Participate in team meetings and share best practices.
- Contribute to the clinical delivery of the service, including completing patient assessments, designing and reviewing care plans, and delivering effective clinical care.
- Assist with daily team coordination, including handovers, work allocation, and managing referrals and messages.
Preferred Skills and Experience:
- Experience in writing accurate records.
- Evidence of providing high standards of clinical care.
- Experience in administering drugs or willingness to attend study days.
- Knowledge of standard setting and clinical audit.
- Community experience or community placement.

- A competitive salary of £31,163 - £37,875 per annum.
