About the Role

We are seeking a highly skilled and experienced pharmacist to join our Medicines Management and Optimisation Team as a Diabetes Pharmacist. This is an exciting opportunity to work with a dynamic and innovative team, providing expert pharmaceutical advice to GPs and other primary care clinicians.

Key Responsibilities
  • Work as part of the multidisciplinary diabetes team in primary care to develop a tailored management plan for identified patients with diabetes.
  • Use expertise to provide evidence-based prescribing advice to ensure effective, appropriate, cost-effective, and safe use of medicines and promote a patient-centred medicines optimisation agenda for patients with diabetes.
  • Undertake clinical medication reviews for each allocated patient, utilising GP computer systems and discuss findings at the MDT so that the required actions can be built into the care plan.
  • Facilitate good prescribing practice in diabetes across the primary and secondary care interface.
  • Deal directly with patients/carers as required, to improve concordance with diabetes treatment in order to achieve better health outcomes.
  • Work with the ICS commissioning team to monitor the delivery of the diabetes aspects of the primary care contract by practices.
  • Actively contribute towards management of the primary care prescribing budget through development of diabetes-related projects.
Requirements
  • Pharmacy degree
  • Registered with General Pharmaceutical Council
  • Relevant clinical diploma/post graduate qualification
  • Evidence of commitment to CPD
  • Specialist diabetes expertise and significant post-registration experience
