Job Summary:
As a Reablement Support Worker, you will play a vital role in providing short-term care and reablement services to patients in their own homes, bridging the gap between hospital and home.
Key Responsibilities:
- Deliver high-quality care and support to patients, promoting independence and well-being.
- Work collaboratively with the D2A Team to ensure seamless transitions and efficient patient flow.
- Conduct thorough assessments and develop personalized care plans to meet individual needs.
- Provide education and support to patients and their families, empowering them to manage their care effectively.
- Maintain accurate records and communicate effectively with the multidisciplinary team.
Requirements:
- Previous care experience is desirable but not essential.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Compassion, empathy, and a patient-centered approach.
