We are seeking a highly motivated and enthusiastic individual to join our dynamic team at Primary Integrated Community Services as a Social Prescribing Link Worker. As a key member of our team, you will play a vital role in providing high-quality care to our patients.
Main Responsibilities- Develop and implement effective social prescribing strategies to promote self-management and improve health outcomes.
- Build and maintain strong relationships with healthcare professionals, community organizations, and stakeholders to ensure seamless referrals and support.
- Provide personalized support to individuals, their families, and carers to empower them to take control of their wellbeing and live independently.
- Collaborate with multidisciplinary teams to develop and implement holistic care plans that address the wider determinants of health.
- Co-produce personalized support plans with service users, introducing or reconnecting them to community groups and statutory services.
- NVQ Level 3, Advanced Level, or equivalent qualifications, or working towards this level.
- Demonstrable commitment to professional and personal development.
- Experience of working directly in a community development context, adult health and social care, learning support, or public health/health improvement.
