We are seeking Band 6 physiotherapists who recognize the transformative power of rehabilitation to join our Home-Based Intermediate Care Team.

This role involves working within a broader integrated care framework, focusing on individuals in the community with intermediate care requirements. Our services include intensive community rehabilitation and strategies to prevent hospital admissions. You will collaborate closely with colleagues in urgent community response and virtual care settings.

As part of a multidisciplinary team, you will engage with Occupational Therapists, Nurses, Consultants, and General Practitioners, while also receiving support from Assistant Practitioners and Rehabilitation Support Workers.

Your direct supervisor will be one of the senior physiotherapists, ensuring a supportive environment where teamwork is emphasized.

Main Responsibilities

The primary responsibilities for these Band 6 positions include:

  • Delivering tailored community rehabilitation services to individuals.
  • Collaborating effectively with a diverse team, local authorities, and community organizations.
  • Providing guidance and training to junior colleagues and support staff in community rehabilitation practices.
  • Infusing compassion and value into every aspect of your work.

Qualifications and Experience

Essential:

  • Experience in community services, rehabilitation, neurology, or managing long-term conditions.
  • Proven ability to work within a physiotherapy team and the wider multidisciplinary team.
  • Experience in supervising and delegating tasks to junior therapists and support staff.
  • Contribution to the clinical education of less experienced staff or physiotherapy students.
  • Prior experience in the NHS is required.
  • A valid driving license is necessary.

Desirable:

  • Involvement in audit and research activities.

Qualifications

Essential:

  • Degree or Diploma in Physiotherapy.
  • HCPC Registration.
  • Portfolio demonstrating continuous professional development.

Desirable:

  • Membership in the Chartered Society of Physiotherapy.

Leadership Skills

Essential:

  • Ability to complete tasks within set deadlines.
  • Demonstrated capability to instruct staff, caregivers, and patients on the safe use of equipment.
  • Resilience in handling challenging conditions, including exposure to unpleasant environments.
