About the Role

We are seeking a highly skilled and experienced Community Mental Health Nurse to join our dynamic team at Gloucestershire Health and Care NHS Foundation Trust. As a key member of our team, you will be responsible for providing high-quality care to individuals with severe mental health needs, working closely with service users, families, and other healthcare professionals to achieve the best possible outcomes.

Key Responsibilities
  • Conduct comprehensive assessments and develop personalized care plans to meet the unique needs of service users.
  • Provide evidence-based interventions and support to optimize service users' ability to remain in the community and access the least restrictive environment.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Participate in clinical governance initiatives to drive quality improvement and excellence in care.
Requirements
  • Registered Nurse Mental Health (RNMH) qualification.
  • Experience in mental health, with a strong understanding of severe mental illness (SMI) and recovery principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
