**Key Responsibilities:**
* Provide direct patient care, including administering medications, wound care, and vital sign monitoring
* Develop and implement individualized care plans to meet the unique needs of each patient
* Collaborate with the interdisciplinary team to ensure comprehensive care and support
* Maintain accurate and up-to-date patient records
**Requirements:**
* Current registration with the Nursing and Midwifery Council (NMC)
* Experience working in a care home setting
* Strong communication and interpersonal skills
* Ability to work effectively in a fast-paced environment
