About the Role

We are seeking a highly skilled and compassionate Social Care Professional to join our Adult Services Mental Health Review Team as a permanent full-time Social Worker. This is an exciting opportunity to make a meaningful difference in the lives of adults with mental health difficulties.

Key Responsibilities
  • Undertake social care reviews and reassessments of need, risk, and capacity.
  • Sourcing and commissioning care, undertaking duty tasks, supporting discharge planning where required, and working as part of the wider multi-disciplinary team.
  • Collaborate and coproduce with internal and external professionals, community services, carers, and the people we support.
