Key Responsibilities- Assist in providing rehabilitation and care services to patients in their own homes, working closely with our Intermediate Care Team.
- Support patients in maintaining their independence and promoting their overall well-being.
- Collaborate with healthcare professionals to develop and implement personalized care plans.
- Provide clerical and administrative support to the team, ensuring accurate and timely documentation of patient information.
- Deliver care services in accordance with organizational policies, procedures, and guidelines, adhering to competencies and delegations from senior team members.
- Report any changes in patient condition to the Operational Lead, ensuring seamless communication and coordination of care.
