We are seeking an experienced and skilled professional to join our team as a Front Door Integrated Discharge Team Lead at Nottingham CityCare Partnership CIC. As a key member of our management team, you will be responsible for leading a multidisciplinary team of healthcare professionals in providing high-quality patient care and services.
Key Responsibilities- Lead and manage a team of healthcare professionals to deliver efficient and effective patient care services.
- Develop and implement strategies to improve patient outcomes and reduce hospital readmissions.
- Collaborate with internal and external stakeholders to ensure seamless care coordination and service delivery.
- Monitor and evaluate service performance, identifying areas for improvement and implementing changes as needed.
