Position Overview

At Shires Health Care, we are seeking a dedicated and skilled Nurse Practitioner who will provide independent and comprehensive care to our patients. This role involves a range of responsibilities from initial patient assessments to the ongoing management of complex health conditions. The ideal candidate will exhibit exceptional clinical judgment and possess advanced diagnostic and prescribing capabilities.

Key Responsibilities

- Operate within the defined scope of practice and adhere to the professional standards set by the regulatory body.

- Evaluate and manage patients with both acute and chronic health issues, including physical and mental health concerns.

- Conduct home visits when necessary for patient assessment and management.

- Perform telephone triage and consultations, addressing patient inquiries effectively.

- Assess, diagnose, plan, implement, and evaluate treatment strategies for patients with complex health needs.

- Order and interpret diagnostic tests, and make appropriate referrals as needed. In urgent medical situations, facilitate hospital admissions and ensure thorough patient handovers.

- Analyze diagnostic findings and take responsibility for patient follow-ups, ensuring that referrals are both appropriate and cost-effective.

- Deliver safe, evidence-based, and individualized patient care in accordance with practice policies, ensuring consistency in care delivery.

- Prescribe medications in line with national guidelines and local formularies, while providing patients and caregivers with essential information regarding medication regimens, potential side effects, and interactions.

About Our Team

As a vital member of the Shires Health Care GP and Nursing team, you will play a crucial role in implementing practice policies and demonstrating clinical leadership.

Qualifications and Experience

Essential Qualifications

  • Minimum of 5 years post-registration experience.
  • Proven experience in autonomous clinical practice, prioritization, and prescribing.
  • Experience in ordering and interpreting diagnostic tests.
  • Demonstrated ability to work collaboratively within a team and exhibit leadership qualities.

Desirable Qualifications

  • Background in General Practice, Out of Hours, or emergency care.
  • Experience managing long-term health conditions.
  • Familiarity with pediatric care.
  • Experience in conducting in-practice diagnostics.
  • Skills in effective staff management and mentorship.
  • Knowledge of infection control practices.

Required Qualifications

  • Registered General Nurse (NMC).
  • BSc or MA in Advanced Clinical Practice or equivalent experience.
  • Independent prescriber status.

Preferred Qualifications

  • Teaching or mentorship certification.
  • Minor illness/injury qualification.
  • Relevant primary care specialist courses.
  • Qualifications in contraceptive coil fitting or cervical smear procedures.
  • Participation in leadership development programs.
