Enhanced Support Worker

We are seeking a highly skilled and compassionate Enhanced Support Worker to join our team at Look Ahead. As a key member of our mental health services, you will play a vital role in supporting individuals with complex needs to achieve their recovery goals.
Key Responsibilities- Build and maintain strong relationships with customers, providing a high level of support and guidance to help them achieve their goals.
- Conduct holistic assessments of new customers, incorporating relevant statutory referral information, to develop effective support plans.
- Key-work the most complex service-users, providing tailored support and interventions to address their specific needs.
- Create and manage risk management and support plans, ensuring ongoing assessment and management of risks associated with customers.
- Provide support and access to relevant services to enable customers to continue their recovery, empowering them to self-manage their medication regimes through individual re-enablement programs.
- Develop and maintain productive relationships with external stakeholders, including partner organizations, to improve service outcomes.
- Upskill staff team members in areas of expertise, promoting a culture of continuous learning and development.
- Adhere to Look Ahead's Health and Safety policies and local protocols, ensuring a safe and healthy environment for customers and staff.
- Up to NVQ Level or equivalent in a relevant area (psychology, health and social care, mental health) or experience in the social care/charity sector.
- Experience building and developing strong professional partnerships with external providers aligned to their specialism.
- IT Literate.
