About the Role

We are seeking a highly skilled and motivated HIU Care Coordinator to join our team at Central London Community Health Trust. As a key member of our community health service, you will play a vital role in coordinating and delivering support services to individuals identified as High Intensity Users (HIUs) of Urgent & Emergency Care services.

Key Responsibilities
  • Identify and review the needs of HIUs, working closely with clinical and other support teams to develop tailored support plans.
  • Coordinate referrals to suitable providers, ensuring seamless transitions and effective care pathways.
  • Track processes and outcomes, using data to inform service improvements and optimize patient care.
  • Provide information and training to local health and social care professionals, promoting awareness and understanding of HIU services.
  • Collaborate with multidisciplinary teams to prioritize care for HIUs, ensuring timely and effective interventions.
Requirements
  • English and Maths GCSE.
  • Specialist knowledge or interest acquired through training and extended courses related to healthcare.
  • Excellent IT skills and a positive attitude towards learning and using your own initiative for problem-solving.
  • Proven record of excellent written and verbal communication and interpersonal skills.
  • Good organizational and time management skills, with the ability to prioritize and manage your workload.
