Job Title: HIU Care Navigator

Job Summary

The HIU Care Navigator is a critical role within our organization, responsible for coordinating and driving the delivery of services to High Intensity Users (HIUs) of Urgent & Emergency Care services. The aim is to improve health outcomes and reduce inequalities by identifying the most appropriate medical and psycho-social support and helping HIUs access it as needed, reducing inappropriate demand on UEC services.

Key Responsibilities

  • Coordinate and drive the delivery of HIU services, including identifying HIUs who need additional support, reviewing their needs, referring them to suitable providers, and tracking processes and outcomes.
  • Work collaboratively with colleagues across health, social care, and the third sector to provide patient-centred care.
  • Support GPs through care coordination for the most vulnerable and complex patients, including intelligent tasking and patient referrals, completing Care Plans, and delivering targeted public health messages.
  • Maintain accurate records and use SystmOne to record all patient interactions, in accordance with GDPR regulations.

Requirements

The ideal candidate will have excellent written and verbal communication and interpersonal skills, with the ability to prioritize and manage their own workload and show initiative to complete tasks independently and with multi-disciplinary teams.

Healthcare Central London Ltd (HCL) is a GP Federation owned by 32 General Practices covering the Central London (Westminster) area, supporting 4 Primary Care Networks (PCNs). We operate several NHS contracts on behalf of our PCNs and are the host employers of a large team of ARRS roles.
