Position Overview

The successful candidate will be a seasoned Nurse with a minimum of three years of experience in a Nurse Practitioner role. Operating within their professional scope, they will deliver comprehensive care to patients, starting from initial history taking through to clinical evaluation, diagnosis, treatment, and ongoing assessment of care. The individual will exhibit proficient clinical judgment and provide expert care, showcasing advanced assessment and diagnostic capabilities for patients within a primary care setting. Critical thinking will be essential in the clinical decision-making process.

Key Responsibilities

Clinical Duties

  • Conduct assessments, establish diagnoses, formulate, implement, and evaluate treatment plans.
  • Manage interventions for patients with intricate health needs.
  • Identify and manage treatment strategies for patients at risk of developing chronic conditions.
  • Diagnose and treat both acute and chronic health issues, incorporating both pharmacological and non-pharmacological treatment modalities.
  • Prescribe and monitor medications for therapeutic effectiveness, ensuring alignment with patient needs and evidence-based practices.
  • Support patients in adhering to prescribed treatments and provide guidance on medication regimens, including potential side effects and interactions.
  • Prioritize health concerns and intervene effectively in urgent or emergency situations, including initiating emergency care when necessary.
  • Encourage patients to adopt health-promoting strategies and self-care principles.
  • Deliver evidence-based care for a diverse range of health conditions.
  • Work autonomously at various practice locations, covering for absent General Practitioners or Nurse Practitioners.

Communication Skills

  • Utilize effective communication techniques to ensure patients are well-informed and consent to treatment.
  • Engage with patients and their families, adapting communication methods to accommodate varying levels of understanding and cultural backgrounds.
  • Proactively address communication barriers to enhance patient interactions.
  • Maintain clear communication within the practice and with external partners.
  • Act as a patient and colleague advocate.
  • Provide information on available support services in an accessible format, recognizing individual patient needs.

Quality Service Delivery

  • Operate within the professional code of conduct as regulated by the relevant nursing authority.
  • Deliver care in accordance with national guidelines and evidence-based practices.
  • Participate in quality governance initiatives to enhance healthcare delivery.
  • Collaborate with clinical teams to improve healthcare quality in response to local and national policies.
  • Engage in shared learning opportunities across the organization.
  • Utilize structured frameworks to manage and learn from patient complaints and clinical incidents.

  • Lead the nursing team, ensuring effective operational support.
  • Collaborate with management to ensure adequate staffing levels and skill mix to meet service demands.
  • Promote the role of the Nurse Practitioner within the primary care setting.

  • Translate national strategies into local practices that align with the values of primary care.

Information Management

  • Utilize accurate coding for data management to facilitate monitoring and auditing.
  • Understand responsibilities regarding data protection and information governance.
  • Ensure the nursing team processes data accurately and efficiently.
  • Communicate financial constraints to the team and discuss strategies for effective operations.

Learning and Development

  • Mentor junior staff, assessing their competencies against established standards.
  • Share knowledge and insights with team members to promote best practices.
  • Educate patients, families, and colleagues in a supportive learning environment.

Qualifications and Experience

  • Relevant nursing or health-related degree.
  • Experience in clinical supervision and mentorship.
  • Independent Nurse Prescriber qualification.
  • Minimum of five years post-registration experience, with at least three years in primary and community nursing.
  • Experience in managing minor illnesses and triage.
  • Leadership in quality initiatives and team management.
