Job Title: Clinical Nurse Specialist - Anticoagulation Expert

About the Role:

The Clinical Nurse Specialist - Anticoagulation Expert will work independently and collaboratively to provide a range of key nurse-led services for patients requiring anticoagulation, reporting to senior coagulation nurse specialists and in line with Trust policies and guidelines.

Main Responsibilities:

  • Provide expert and holistic care through assessment, treatment, education, and follow-up services for patients requiring anticoagulation and thrombosis management.
  • Deliver safe and effective care in a timely manner through a range of clinical services within inpatient and outpatient settings.
  • Manage patient referrals to the anticoagulation and thrombosis services, prioritizing patient safety and effective care.
  • Work in the nurse-led DVT service, providing patient assessment, examination, ordering diagnostics, and initiating therapy.
  • Participate in multidisciplinary team (MDT) meetings and ward rounds to present patients and co-design effective care plans.
  • Contribute to the management of patients with immune thrombocytopenia (ITP).
  • Ensure clear and concise verbal and written communication with patients, the clinical team, and key stakeholders involved in patient care.
  • Manage the clinical workload with a thoughtful and strategic approach, ensuring prioritization to optimize patient safety.
  • Recognize own limitations in skills and knowledge and escalate to clinical colleagues appropriately.
  • Ensure clinical governance standards are upheld at all times.
  • Act as a role model, working in a courteous, patient-focused manner.
  • Provide clinical leadership and deputize for the senior CNS when necessary.

Education and Training:

  • Take responsibility for own professional and personal development.
  • Actively participate in departmental education programs and opportunities.
  • Undertake any training identified by line manager as necessary for the role.
  • Identify opportunities for education and training and work with the senior nurse to deliver these.
  • Deliver education and information to patients and their carers to help them make informed decisions.
  • Act as a resource by sharing best practice with colleagues within the Trust and other services to improve patient care.
  • Support and assist in training of new staff to the department.

Research and Service Development:

  • Undertake quality improvement work directly benefiting the patient group.
  • Participate in research and development projects within the department, undertaking any necessary training.
  • Commit to a culture of continuous improvement and look for solutions to problems.

Information Systems and Data Management:

  • Abide by the Trust information governance policy.
  • Be an active user of the information systems, looking for ways to improve efficiency, patient safety, and patient experience.
  • Accurately and contemporaneously record patient information in clinical records.

Clinical Governance:

  • Actively support the process of risk management, ensuring incidents are reported appropriately and help identify ways of preventing errors and accidents to both patients and staff.
  • Maintain standards of hygiene and cleanliness within the clinical area, taking action as appropriate and reporting deficits as necessary.
  • Attend and contribute to departmental meetings as required.
  • Work within and accept responsibility for maintaining agreed levels of clinical competence.

Policy and Process:

  • Adhere to Trust and departmental policies and procedures.
  • When required, review and update policies, SOPs, and procedures.

Person Specification:

  • Education:
    • Evidence of post-qualification education and development.
    • Successful completion of an Advanced Assessment Skills Course (desirable).
  • Experience:
    • Significant post-registration experience as a Band 5 RGN in a relevant specialty.
  • Skills:
    • Ability to organize and manage day-to-day departmental activities.
    • Ability to relate and communicate with patients and staff at all levels and from different professions.
    • Ability to utilize research and guidelines in clinical practice.
    • Flexibility - able to adapt to ensure achievement of objectives within constantly changing situations and environments.
    • Patient-centered - committed to ensuring a positive hospital experience for patients and their relatives/carers.
