Job Summary
We are seeking a highly skilled Clinical Nurse Specialist to join our team at King's College Hospital NHS Foundation Trust. The successful candidate will be responsible for providing expert care to patients requiring anticoagulation and thrombosis management.
Main Responsibilities
- Safe management of outpatients taking warfarin and other vitamin K antagonists, including checking INR results, titrating warfarin doses, and arranging follow-up.
- Safe management of patients taking other anticoagulants (DOACs, LMWH, etc.) by undertaking in-person and telephone consultations to initiate and manage patients on anticoagulants and supporting patients outside the clinic setting.
- Management and prioritization of patient referrals to the anticoagulation and thrombosis services.
- Work in the nurse-led DVT service, providing patient assessment, examination, ordering diagnostics, and initiating therapy.
- Participation in the anticoag/thrombosis MDT weekly meeting to present patients and co-design effective care plans.
- Contribution to twice-weekly MDT ward rounds by reviewing patients and contributing to their care planning.
- Participation in the management of patients with immune thrombocytopenia (ITP).
- Ensure clear and concise verbal and written communication with patients, the clinical team, and key stakeholders involved in the patient's care.
- Manage the clinical workload with a thoughtful and strategic approach, ensuring prioritization to optimize patient safety.
- Recognize own limitations in skills and knowledge and escalate to clinical colleagues appropriately.
- Ensure clinical governance standards are upheld at all times.
- Act as a role model, working in a courteous, patient-focused manner.
- Provide clinical leadership and deputize for the senior CNS when necessary.
Education and Training

- Actively participate in departmental education programs and opportunities.
- Undertake any training identified by line manager as necessary for the role.
- Identify opportunities for education and training and work with the senior nurse to deliver these.
- Deliver education and information to patients and their carers to help them make informed decisions.
- Act as a resource by sharing best practice with colleagues within the Trust and other services to improve patient care.
- Support and assist in training of new staff to the department.
Research and Service Development
- Undertake quality improvement work directly benefiting the patient group.
- Participate in research and development projects within the department, undertaking any necessary training.
- Commit to a culture of continuous improvement and look for solutions to problems.
Information Systems and Data Management
- Abide by the Trust information governance policy.
- Be an active user of the information systems, looking for ways to improve efficiency, patient safety, and patient experience.
- Accurately and contemporaneously record patient information in clinical records.
Clinical Governance
- Actively support the process of risk management, ensuring incidents are reported appropriately and help identify ways of preventing errors and accidents to both patients and staff, resolving any shortfall as appropriate.
- Maintain standards of hygiene and cleanliness within the clinical area, taking action as appropriate and reporting deficits as necessary.
