Residential Youth Support Facilitator

Polaris Community is in search of dedicated and motivated Residential Youth Support Facilitators to join our team on a casual basis, providing coverage for shifts as needed. This role offers the flexibility to work hours that fit your schedule without any contractual commitments.
Our mission is to create a positive and enduring impact on young individuals transitioning from care to independence. We offer them not only accommodation but also essential support to help them achieve independent living and educational success.
As a Residential Youth Support Facilitator, your responsibilities will include delivering a tailored, high-quality service to young people residing in their individual flats or shared accommodations.
The young individuals supported by Polaris Community are often vulnerable, and it is your duty to ensure that the environment in which you work is safe, welcoming, and conducive to their well-being.
KEY RESPONSIBILITIES:
- Implement person-centered risk assessments and support plans, collaborating with other support staff for comprehensive handovers.
- Monitor changes in behavior, documenting observations and reporting any shifts in the support needs of the young individuals.
- Foster positive relationships with young people, colleagues, and external professionals through effective communication, actively seeking collaborative solutions to enhance their experiences.
- Ensure that continuous support is provided to young individuals, maintaining the same standards as day staff, with a focus on the safety of vulnerable individuals.
- Conduct necessary health and safety tasks, including cleaning duties, in accordance with established policies and procedures.
- Engage with external agencies such as law enforcement and emergency services when required.
- Assist young individuals in accessing medical care or legal support when necessary.
QUALIFICATIONS:
- Experience in supporting vulnerable young individuals.
- Flexibility in availability, including the potential for weekend work.
- Possession of a Residential Childcare/Families Practitioner Diploma, Children and Young People's Workforce, NVQ Level 3 in Health & Social Care, or a willingness to pursue equivalent qualifications.
- A valid driving license and access to a vehicle for travel purposes.
- Proficient computer skills.
