Position Overview

We are currently seeking compassionate and experienced Licensed Practical Nurses (LPNs) to provide personalized care to patients in their homes. As a Private Duty Nurse LPN, you will play a vital role in improving the lives of individuals by delivering high-quality nursing services tailored to their unique needs.

Responsibilities

  • Deliver and coordinate skilled nursing care under the supervision of a Registered Nurse, adhering to physician orders and established care plans.
  • Continuously assess patient conditions, document observations, and promptly report any changes in status to the supervisor or physician.
  • Implement and update the nursing care plan, ensuring that all interventions are aligned with patient goals and preferences.
  • Educate patients and families on disease processes, self-care techniques, and preventive strategies to promote optimal health outcomes.
  • Maintain accurate and detailed records of all nursing activities, assessments, and treatments in accordance with regulatory guidelines.

Qualifications

  • Graduate of an accredited school of nursing.
  • Current unrestricted state license as a Licensed Practical Nurse (LPN).
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care standards.
