About the Role

We are seeking a highly motivated and experienced Nutrition Specialist to join our team at Everyone Health Ltd. As a Nutrition Specialist, you will play a key role in delivering high-quality nutrition advice and support to adults across Staffordshire.

Key Responsibilities

  • Provide comprehensive assessments of nutritional needs and develop personalized care plans to address nutritional issues.
  • Evaluate nutritional needs and ongoing support to patients in line with NICE guidance, delivering high-quality patient-centered care in group and 1-2-1 settings.
  • Support the development, delivery, evaluation, and quality assurance of the integrated service, providing key nutritional expertise to the multidisciplinary team.
  • Facilitate and empower patients to change behavior and adopt positive lifestyle changes through health education and high-quality nutritional counseling.
  • Attend multi-professional meetings to provide nutrition input regarding patient management, respecting professional roles to ensure a supportive and proactive multidisciplinary team.
  • Support community engagement, networking, publicity, and promotional activity to raise the profile of the service and identify target populations.
  • Monitor and analyze patient performance outcome measures, ensuring contractual KPIs are achieved and an excellent service is demonstrated.
  • Ensure rigorous and regular data inputting to maintain accurate records, complying with EH procedures, Information Governance, and Data Protection.

Requirements

  • Minimum of 1 year's experience working with adults in community settings, including community, primary care, and in-patient settings.
  • Experience of working with adults over 18 years of age in 1-2-1 and group environments to support positive lifestyle change.
  • Experience of delivering high-quality patient nutritional support through 1-2-1 and group settings.
  • Relevant degree/postgraduate degree in nutrition and membership of the Association for Nutrition (AfN) or BDA.
