Community Dietitian - Band 5 Rotational Post

We have an exciting opportunity for a Band 5 dietitian to join our Specialist Adult Dietetic Service covering South Staffordshire. This is an excellent opportunity to work within a friendly team in a dynamic NHS community setting and would be suitable for a newly qualified dietitian (or awaiting registration) or someone who already has some experience of working as a Band 5 dietitian.
Our team deliver a dietetic service for a wide variety of patients including nutrition support, home enteral feeding, gastroenterology, diabetes, mental health and those with complex weight management needs. The post holder will have the opportunity to work and rotate across the following teams:
Weight management and Diabetes
Nutrition Support and Home Enteral Feeding
Mental Health (in and outpatients)
Gastroenterology
The postholder’s role, dependant on where they are working, may include face to face or telephone clinics, home visits, group work with patients or training for staff.

To be professionally accountable and responsible for all aspects of own work.
To assess new service user referrals and care/support needs, using analysis of patients biochemistry, anthropometry, medical & dietary history and behaviour to plan specific dietary therapy interventions.
To formulate, develop, implement and evaluate programmes of treatment /care/support for individuals, families and carers. The care/support plans may include the following:
Promoting independence/self management and maintaining safety
Increased functional ability
Improvement in health and wellbeing including mental health
Develop and independently run dietetic therapy groups.
Interventions in disease/condition management
Prevention and reduction of health inequalities
Admission avoidance
Early supported discharge
Case management/ treatment/ care plans
Palliative and end of life care
To use professional knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider each service user’s personal circumstances and preferences in order to implement and adjust programmes of care and intervention.
To maintain sensitivity at all times to the emotional needs of the service user and their carers offering reassurance and empathy. This may be in regard to the nature/prognosis of their condition and implications of this.
To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be managed.
To maintain contemporaneous and accurate records as required by the Partnership Trust and in line with professional standards (HCPC). This includes self management and supervision of identified team members, giving guidance and direction where appropriate.
