Position: Community Rehabilitation Specialist

Note: This role is primarily focused on physiotherapy, but applications from Occupational Therapists are welcomed.

Band 6 | Permanent | Full-time hours

The Integrated Urgent Care Team is a collaborative health and social care initiative dedicated to implementing a 'Home First' approach, aimed at avoiding unnecessary admissions and facilitating assessments post-discharge.

Our transformation of urgent care services centers around the Integrated Urgent Care Team (IUCT), which is committed to delivering exceptional community healthcare across Tameside & Glossop. Key Responsibilities

As a Community Rehabilitation Specialist, you will:

  • Conduct comprehensive holistic assessments for individuals referred to the Integrated Urgent Care Team.
  • Provide physiotherapy assessments, treatments, and guidance to adult patients in community settings.
  • Lead the assessment, diagnosis, and development of physiotherapy treatment plans, ensuring integration into the patient's overall care program.
  • Train patients, caregivers, families, and staff from other agencies to ensure effective physiotherapy treatment.

Job Responsibilities

Main Duties and Responsibilities

  • Function as an independent practitioner.
  • Adapt treatment methods for home care settings, working autonomously.
  • Organize and prioritize your patient caseload effectively.
  • Deliver high-quality physiotherapy services to patients in their homes or residential facilities.
  • Ensure the safety and well-being of patients, colleagues, and yourself during treatment, adhering to professional standards and health regulations.
  • Assess, prescribe, and provide a variety of equipment and aids to support daily living.
  • Maintain professional competency through continuous professional development (CPD) activities.

Communication Skills

In this role, you will:

  • Obtain consent from patients and collaboratively set treatment goals.
  • Communicate sensitively with patients and families, particularly those experiencing distress due to health challenges.
  • Provide expert advice on patient care to other health and social service professionals.
  • Train and advise other professionals, caregivers, and families on physiotherapy-related matters.
  • Effectively communicate with various health sector and external agencies for information sharing and problem-solving.

Qualifications and Experience

Essential:

  • BSc Honours in Physiotherapy or Occupational Therapy (or equivalent)
  • HCPC Registration
  • Experience at Band 5 level
  • Ability to travel across various locations

Knowledge and Skills

Essential:

  • Excellent communication skills
  • Proficient computer skills
  • Strong interpersonal abilities
  • Ability to perform under pressure
  • Good organizational and planning skills
  • Understanding of current legislation and policies related to Intermediate Care services
  • Evidence of ongoing professional development

Desirable:

  • Experience in supporting audits and research within a team
