Job Description

Sonderwell is seeking an experienced Clinical Lead Nurse to join our team in the Cornwall region. As a Clinical Lead Nurse, you will be responsible for leading, supporting, and facilitating the nursing and medical needs of service users with their commissioned care.

Key Responsibilities:
  • Deliver High-Quality Client-Centered Care: Conduct care assessments, write care plans, and manage competencies to ensure personalized service outcomes.
  • Support Leadership Team: Collaborate with the Senior Nurse Manager and Registered Manager to provide guidance and leadership to care and support staff.
  • Maintain Professional Development: Stay up-to-date with national standards, legislative requirements, and best practices to enhance professional skills and knowledge.
  • Ensure Compliance: Perform duties in accordance with national standards, follow legislative requirements, and adhere to relevant regulations.
Opportunities and Challenges:
  • Develop Individualized Care Plans: Work with service users, families, and healthcare professionals to create patient-focused, realistic, and achievable care plans.
  • Monitor Client Outcomes: Proactively use expertise to guide and implement changes to ensure positive client and family-centered outcomes.
  • Undertake Initial Assessments: Conduct holistic nursing assessments in partnership with service users, families, and carers, involving other statutory, voluntary, or third-party sector providers as necessary.
  • Manage Regional Care Delivery Team: Oversee day-to-day operations, working with committed carer teams to maintain required standards.
  • Ensure Recording Systems: Maintain accurate and up-to-date recording systems and organizational documentation.
  • Motivate and Manage Team: Embed company values and behaviors, promoting a culture of pride in work among nursing and care teams.
  • Support Field-Based Care Workers: Provide regular supervisions, competency assessments, and appraisals to field-based care support workers and nurses.
  • Report and Improve: Report against targets and standards, implementing action plans for service improvements that positively impact clients, service quality, and business development.
Working with Others:
  • Collaborate with Central Support: Work with HR and other departments to motivate and support carers and nurses through informal check-ins, formal supervision, and appraisals.
  • Oversee Rota Development: Ensure field staff are trained, assessed, and signed off as competent prior to lone working.
  • Protect Service User Welfare: Promote and protect the welfare of service users, working in cooperation with multidisciplinary teams to maximize opportunities.
