About the Role

We are seeking an experienced Clinical Lead Nurse to join our team in the Cornwall region. As a Clinical Lead Nurse, you will be responsible for leading, supporting, and facilitating the nursing and medical needs of service users with their commissioned care.

Key Responsibilities
  • Delivering Personalised Care: Delivering high-quality, client-centred care outcomes, conducting care assessments, and writing care plans to ensure the best possible outcomes for our service users.
  • Leadership and Support: Providing leadership and support to the care and support staff, working closely with the Senior Nurse Manager and Registered Manager to ensure the highest standards of care are maintained.
  • Professional Development: Maintaining your own professional skills and undertaking required training and development to ensure you remain up-to-date with the latest best practices and regulations.
  • Compliance and Quality: Ensuring that all care is delivered in accordance with national standards, legislative requirements, and accepted best practices.
What You'll Achieve
  • Develop Individualised Care Plans: Working with service users, families, and healthcare professionals to develop individualised care plans that are patient-focused, realistic, and achievable.
  • Monitor Client Outcomes: Monitoring client and family-centred outcomes, using your expertise to guide and implement changes to ensure the best possible outcomes.
  • Initial Assessments: Conducting initial holistic nursing assessments in partnership with service users, families, and carers, involving other statutory, voluntary, and third-party sector providers as necessary.
  • Team Management: Day-to-day oversight and management of the regional care delivery team, working with committed carer teams to maintain all required standards.
  • Recording and Documentation: Ensuring that all recording systems and organisational documentation are of a high standard and up-to-date.
  • Team Motivation and Management: Motivating and managing your nursing and care team, embedding the company's values and behaviours and a culture of 'pride' in their work.
  • Reporting and Improvement: Reporting against targets and standards, putting in place action plans for service improvements that have a positive impact on clients, service quality, and business development.
Working with Others
  • Collaboration and Support: Working in collaboration with Central Support, including HR, to motivate carers and nurses through informal check-ins, formal supervision, and appraisals.
  • Rota Development and Implementation: Overseeing the development and implementation of rotas, ensuring that field staff are appropriately trained, assessed, and signed off as competent prior to lone working.
  • Service User Welfare: Being responsible for promoting and protecting the welfare of our service users.
  • Multi-Disciplinary Teamwork: Working in cooperation with members of the multidisciplinary teams to maximise opportunities for our service users, identifying information, concerns, and ideas for improvement, and sharing them with the senior management team of Sonderwell.
About You

We are looking for an experienced nurse who can blend clinical nursing skills with coaching and management of people and services. Experience of working in the community would be ideal, and you will have experience of tracheostomy, peg, and catheter care, passing on your knowledge to others in the package. You will be happy working with both adults and paediatric care, and we will provide training where required.
