We are seeking a highly skilled and experienced Rapid Response Nurse to join our Complex Care team at Superior Healthcare. As a key member of our team, you will be responsible for providing high-quality, person-centered care to clients with complex health needs in their own homes.
Key Responsibilities:
- Deliver exceptional care to clients with complex health needs, including those with life-limiting conditions, spinal injuries, and brain injuries.
- Work collaboratively with clients, families, and healthcare professionals to develop and implement personalized care plans.
- Provide training and support to Healthcare Support Workers and Nurses to ensure continuity of care.
- Contribute to continuous improvement initiatives and maintain quality care delivery standards.
- Ensure compliance with safeguarding procedures, report concerns promptly, and complete relevant documentation.
