We are seeking a highly skilled and compassionate Registered Nurse to join our team at PDA Physicians Dialysis Acquisitions, Inc. as a Dialysis Nurse - Patient Care Specialist. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities- Provide high-quality patient care in an outpatient dialysis setting, working closely with a multidisciplinary team to deliver exceptional outcomes.
- Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
- Collaborate with healthcare professionals to ensure seamless transitions of care and optimal patient outcomes.
- Participate in quality improvement initiatives to drive excellence in patient care and safety.
- Contribute to a positive and supportive team environment, fostering open communication and a culture of respect.
- Current Registered Nurse (RN) license in the state of practice.
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from an accredited diploma program may be substituted for nursing degree.
- Experience in intensive care unit (ICU), critical care unit (CCU), emergency room (ER), or medical surgical (Med Surg) settings is highly desirable.
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) certification is a plus.
