About the Role

We are seeking a skilled and compassionate Advanced Healthcare Assistant to join our Community Nursing Team at Kent Community Health NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality patient care and support to individuals in their own homes and in residential care settings.

Key Responsibilities
  • Provide personalized nursing care to patients with complex and long-term conditions, working closely with our multidisciplinary team to develop and implement care plans.
  • Conduct regular assessments and reviews to ensure patients receive the best possible care and support.
  • Work collaboratively with patients, families, and carers to promote independence, dignity, and well-being.
  • Participate in ongoing training and professional development to stay up-to-date with the latest clinical practices and guidelines.
  • Contribute to the development and implementation of care pathways and protocols to improve patient outcomes.
Requirements
  • Relevant level 3 qualification (NVQ/ QCF Diploma or certificate of care / Advanced apprenticeship) or equivalent.
  • Minimum of 2 years' experience as a Healthcare Assistant/ Worker in a health or social care setting.
  • Experience of working as part of a team and using Microsoft Office and other IT packages.
  • Awareness of developments in the NHS and knowledge of up-to-date clinical practice.
  • Sound communication and interpersonal skills, with the ability to organize own work and manage own time effectively.
