About the Role

We are seeking an experienced Advanced Healthcare Assistant to join our Community Nursing Team in Maidstone. As a key member of our team, you will provide high-quality care to patients in their own homes, working closely with our nursing staff to deliver personalized care plans.

Key Responsibilities
  • Provide care to patients in their own homes, following agreed care plans and protocols.
  • Work collaboratively with our nursing team to deliver high-quality care and support to patients.
  • Monitor patient progress and report any concerns or issues to the nursing team.
  • Contribute to the development and implementation of care plans, working with patients, families, and other healthcare professionals.
  • Provide education and support to patients and their families, promoting independence and self-care.
Requirements
  • Relevant level 3 qualification (NVQ/ QCF Diploma or certificate of care / Advanced apprenticeship) or equivalent.
  • Minimum of 2 years' experience as a Healthcare Assistant/ Worker in a health or social care setting.
  • Experience of working as part of a team and using Microsoft Office and other IT packages.
  • Valid driving licence and access to a car for work purposes.
