Advanced Practitioner Social Worker - Hospital Team (Adults)

As an Advanced Practitioner Social Worker with Somerset County Council, you will play a key role in overseeing a team of Social Workers in a healthcare environment. Your primary responsibility will be to swiftly identify solutions to assist individuals who no longer require acute hospital treatment.

  • Have a Social Work degree qualification and be registered with Social Work England.
  • Manage and oversee a team of staff who will tailor solutions around individuals' needs when they no longer require an acute hospital admission.
  • Utilize performance and hospital-related data to prioritize resources as needed.
  • Work closely with community, health, and third sector partners to provide better outcomes for individuals and reduce readmission.
  • Collaborate with key partners to facilitate improved service provision, team training, and recruitment.
