Job Description

The role of the Field Care Supervisor is to provide high-quality care support services by carrying out a range of functions in the field, including assessment and review of individual services, and practical supervision, instruction, and support of care workers.

Working closely with Care Coordinators to plan realistic and effective care worker rotas, taking into account the individual needs of service users and the skills and attributes of care workers. Regularly reviewing individual services, including repeated and renewed needs and risk assessments, and participating in multi-disciplinary reviews of service users' needs as required.

Field Care Supervisors are expected to spend most of their working time in the community, carrying out their primary functions, but their role will also involve some office-based administrative work.

Qualifications

To be successful in this role, you will need experience in carrying out initial risk and needs assessments for individual service users, either in their homes or in other settings such as hospitals or residential/nursing homes.

You will work in partnership with service users, their families, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of holistic packages of individual care support.
