Job Overview

The Major Trauma Coordinator will play a pivotal role in the multidisciplinary After Trauma Team, coordinating the patient pathway from admission to discharge for patients with highly complex traumatic injuries.

Main Responsibilities
  • Coordinate the care pathway of patients requiring on-going inpatient and community rehabilitation following a traumatic injury.
  • Work across the hospital to ensure seamless care coordination, liaising with families, neuro navigators, and other healthcare professionals.
  • Facilitate the transfer of patients to appropriate interim facilities, liaising across the trauma networks and rehabilitation units.
  • Contribute to data collection and maintaining patient lists, as well as participating in the Trauma Follow-up clinic.
  • Act as a keyworker, liaising with relatives and serving as a point of contact for patients who have been discharged.
