About the Role

We are seeking a highly skilled and dedicated Health Visitor to join our team at Hunter AHP. As a Health Visitor, you will play a vital role in promoting the health and wellbeing of children and families in our community.

Key Responsibilities:

  • Manage caseloads and deliver the Healthy Child programme to ensure high-quality service to families, individuals, and groups in the community.
  • Work collaboratively within multi-agency settings to champion health and wellbeing, promoting physical, social, and psychological well-being for all.
  • Develop and maintain strong relationships with families, individuals, and community groups to provide support and guidance on health-related issues.
  • Conduct home visits and assessments to identify health needs and provide appropriate interventions.
  • Work closely with other healthcare professionals to ensure seamless care and support for families and individuals.
  • Participate in ongoing professional development to stay up-to-date with current health agendas and best practices.

Requirements:

  • SCPHN Health Visitor Post Graduate Diploma or Certificate
  • Current NMC Registration
  • Excellent written and verbal communication skills
  • Proven experience in multi-agency working and child protection
  • Commitment to ongoing professional development
  • Knowledge of current health agendas relating to children, families, and health visiting
  • Ability to travel to various locations for work duties
