Job Overview

We are seeking a highly skilled and dedicated Community Psychiatric Nurse to join our team at West London NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing specialist assessments, reviews, and nursing interventions to service users accessing our Cognitive Impairment and Dementia Service (CIDS).

Main Duties and Responsibilities
  • Conduct comprehensive assessments of individuals and their families to identify their needs and develop personalized care plans.
  • Work collaboratively with other healthcare professionals to ensure the successful implementation and roll-out of care for dementia patients.
  • Provide supervision and guidance to junior staff and trainees as needed.
  • Act as a core member of the multidisciplinary team, contributing to the development and implementation of care plans.
  • Deliver care under the Care Programme Approach, working in partnership with individual service users and their families and carers to ensure the delivery of the care plan.
  • Administer and monitor medication, including depots, prescribed to individual service users, providing information and advice within NMC guidelines and the Trust's Medicines Code.

Person Specification

We are looking for a qualified and experienced Community Psychiatric Nurse who possesses the following skills and qualities:

  • Professionally qualified and registered Nurse with current NMC registration in Mental Health/LD/Adult Nursing.
  • Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
  • Experience of supervising staff and managing clinical caseloads.
  • Ability to work within a multidisciplinary team and contribute to the development and implementation of care plans.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with other healthcare professionals.
