Job Summary

Thames Reach is seeking a highly skilled and compassionate professional to join our team as a Lead Worker. As a key member of our Outreach team, you will play a vital role in helping individuals who are rough sleeping to find routes away from the street.

Key Responsibilities

  • Provide direct support to individuals who are rough sleeping to enable them to move away from the streets and into accommodation.
  • Work closely with partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation.
  • Assess individual support needs and develop support plans to help clients achieve their goals.
  • Provide guidance and supervision to junior team members to ensure the delivery of high-quality services.
  • Contribute to the development and implementation of outreach strategies to prevent homelessness and support vulnerable individuals.

Requirements

  • Experience working with individuals who have rough slept or vulnerable adults and supporting them to access services.
  • Excellent organisational and time management skills to remain strongly outcome-focused.
  • Strong IT skills with the ability to use a range of software applications, including Outlook, online recording systems, Excel, and Word.
  • Ability to work independently and as part of a team to achieve shared goals.
  • Commitment to the values and principles of Thames Reach, including a passion for delivering high-quality services to vulnerable individuals.
