About the Role

We are seeking a highly skilled and experienced Occupational Therapist to join our South Community Therapy Team as a Senior Community Rehabilitation Specialist. This is an exciting opportunity to work in a dynamic and supportive environment, delivering high-quality care to patients in their own homes and communities.

Key Responsibilities
  • Provide specialist assessments and reviews to improve or maintain quality of life for patients with a variety of conditions.
  • Develop and implement personalized care plans to support patients in achieving their goals and improving their functional abilities.
  • Work collaboratively with multidisciplinary teams to coordinate care and ensure seamless transitions between services.
  • Supervise and support junior staff members to develop their clinical skills and knowledge.
  • Contribute to the development of clinical policies and procedures to ensure best practice in community rehabilitation.
Requirements
  • Professional qualification in Occupational Therapy and current registration with the Health Professions Council.
  • Minimum of 2 years postgraduate experience as an Occupational Therapist, with a strong background in community rehabilitation.
  • Experience of working in a range of clinical specialties and ability to manage complex situations.
  • Valid driving license and ability to travel to patients' homes and community locations.
