Job Title: PCN Paramedic

We are seeking a highly skilled and experienced Paramedic to join our team at Swan Medical Centre. As a PCN Paramedic, you will play a vital role in delivering high-quality patient care and contributing to the success of our Primary Care Network.

About the Role

The successful candidate will be responsible for working closely with GPs and other healthcare professionals to provide patient-centered care. You will be expected to perform specialist health checks, reviews, and investigations, as well as provide anticipatory care plans and lead certain services.

Key Responsibilities
  • Work as part of a multi-disciplinary team to deliver high-quality patient care
  • Perform specialist health checks and reviews, and provide anticipatory care plans
  • Lead certain services, including monitoring blood pressure and diabetes risk in elderly patients
  • Communicate effectively with patients, families, and carers, and maintain accurate and contemporaneous health records
  • Collaborate with other healthcare professionals to improve patient outcomes and contribute to the success of the PCN
Requirements
  • Degree in Paramedic Science or equivalent
  • Professional registration with HCPC and no current issues under investigation
  • Driving Licence
  • Mentoring and coaching experience
  • Knowledge of NICE guidelines and evidence-based practice
  • Excellent interpersonal and organisational skills
