Job Title: Apprentice District Nurse

Join our team at Your Healthcare CIC as an Apprentice District Nurse, where you will play a vital role in delivering high-quality healthcare services to our community.

Key Responsibilities:
  1. Health Visiting Service: Support the provision and maintenance of a comprehensive health visiting service to all clients for a defined population.
  2. Health Needs Assessment: Assess the health care needs of individuals and families within the defined caseload and develop, implement, and evaluate programmes of care.
  3. Safeguarding Children: Maintain the safety and protection of children by following Local Safeguarding Board Procedures and develop services that promote their safety and well-being.
  4. Health Promotion: Promote good health care by consulting and advising individuals and families, referring and liaising with other professionals and agencies as appropriate.
  5. Home Visits: Undertake home visits to support, advise, and inform parents, especially targeting vulnerable children and their families.
  6. Clinical Governance: Participate in Clinical Governance and Audit activities to ensure the highest quality of practice is maintained.
  7. Public Health Role: Develop a family-centred public health role in line with the local and national Public Health Agenda.
  8. Community Health Needs: Identify community health needs in partnership with other agencies to develop and deliver appropriate services and evaluate effective outcomes of intervention.
  9. Child Health Surveillance: Participate in the child health surveillance programme and actively promote the Public Health Agenda.
  10. Health Education: Engage in health promotional and health educational activities in various community settings.
  11. Professional Development: Maintain an up-to-date knowledge base and skill development reflecting local needs and an evidence-based approach to practice.
  12. Team Working: Create, maintain, and enhance effective working relationships within the team and with other staff and agencies to promote interdisciplinary team working.
  13. Reporting: Submit written reports as determined by Your Healthcare and the Local Authority.
  14. Learning Environment: Promote, develop, and foster an environment conducive to learning for students and clinicians.
  15. Culture of Reflective Practice: Develop a culture of reflective practice and self-awareness within the Children and Family Services Team.
  16. Policy Development: Identify the need to change policies and influence the change process.
  17. Information Systems: Utilize automated information systems to improve quality and coordination of services and enable faster and more certain communication within Your Healthcare.
