Key Responsibilities:
- Lead a multidisciplinary team to provide high-quality care and support to service users.
- Develop and implement care plans that promote independence and well-being.
- Collaborate with healthcare professionals to ensure seamless care delivery.
- Manage and coordinate community visits and activities.
- Provide emotional support and guidance to service users and their families.
- Contribute to the development and implementation of policies and procedures.
- Participate in quality improvement initiatives and audits.
Requirements:
- Experience as a Team Leader or in a similar role in a healthcare setting.
- Strong leadership and communication skills.
- Ability to work effectively in a fast-paced environment.
- Knowledge of learning disability and Autism care principles.
- Valid UK driving license (desirable).
- NVQ 3 in Health and Social Care (or equivalent).
