Job Summary:

We are seeking a dedicated and passionate Key Worker to provide intensive support to young people aged 16-25 with Special Educational Needs and Disabilities (SEND). The successful candidate will develop personalised action plans, coordinate with professionals, and support young people in their educational and personal development journeys.

Key Responsibilities:

  • Provide individualised support to young people with SEND, focusing on their strengths, needs, and aspirations.
  • Develop and implement tailored action plans to promote educational and personal development.
  • Support access to volunteering, work experience, and educational opportunities that align with the young person's goals and aspirations.
  • Act as the primary point of contact for young people and their parents/carers, providing regular updates and progress reports.
  • Coordinate with a team of professionals to deliver holistic support, ensuring a seamless and effective service.
  • Attend and contribute to multi-agency meetings, promoting collaboration and information sharing.
  • Ensure high attendance rates and timely completion of assessments and action plans, maintaining accurate records and ensuring compliance with safeguarding and data protection protocols.
  • Engage in 1-to-1 activities tailored to young people's needs, including vocational, PSHE, and enrichment activities that promote personal growth and development.
  • Assist in exploring next steps beyond the programme, ensuring smooth transitions and supporting young people in their future plans.
