About the Role

We are seeking an experienced and skilled Senior Community Nurse to join our District Nursing Service in Halton Borough. As a Senior Community Nurse, you will play a key role in providing high-quality patient care and leading a team of nurses to deliver excellent outcomes.

Key Responsibilities
  • Provide clinical leadership and guidance to the District Nursing Team, ensuring that patients receive high-quality care and treatment.
  • Develop and implement effective care plans, working closely with patients, carers, and other healthcare professionals to achieve optimal outcomes.
  • Conduct comprehensive assessments, identify patient needs, and develop individualized care plans to address these needs.
  • Lead and manage a team of nurses, providing guidance, support, and supervision to ensure that patients receive high-quality care.
  • Collaborate with other healthcare professionals, including GPs, hospital staff, and social services, to ensure seamless care coordination and patient-centered care.
  • Participate in clinical audits and quality improvement initiatives to ensure that care is evidence-based and patient-centered.
  • Develop and maintain effective relationships with patients, carers, and other stakeholders to ensure that care is patient-centered and responsive to their needs.
Requirements
  • Registered Nurse with a valid NMC registration.
  • Postgraduate qualification in a relevant field or working towards.
  • Experience of working in a district nursing service or a similar community healthcare setting.
  • Proven leadership and management skills, with experience of leading a team of nurses.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours and shifts.
